OBJECTIVE: In contrast of the impressive advances made in somatic research of erectile dysfunction, psychogenic erectile dysfunction is mostly treated as a monolithic block. We evaluatetd the erectogenic power of imagination of divergent self-concepts in men with psychogenic erecile dysfunction.

MATERIALS AND METHODS: 32 men with erectile dysfunction of no known organic cause with a mean age of 46 years (range 26-63 y) were entered in a placebo controlled study in which the erectogenic power of imagination and a placebo were compared. All patients were assessed using a standard protocol which included history and examination, colour flow duplex doppler ultrasonography, and werde indicated Rigiscan nocturnal penile tumescence testing and / or dynamic infusion caversonometry and caversonography.

METHOD AND BACKGROUND: The new method of imagination of divergent self-concepts developed by Obenaus works on the principle of following integration of two concepts of the beginning into a unified third concept. These concepts are expressed by imagined pictures wich finally are painted or drawn by the patients. The third picture containes positive changes of feelings and characteristics of the patient.

RESULTS: 32 patients were rondomised, two patients never returned for follw-up, six patients wanted to have other treatment options, so 24 patients are evaluable for effect assessment. There was significant difference in the subjective results by imagination of divergent self-concepts (60% increase of potence) compared to placebo (16% increase of potence).

CONCLUSION: Imagination of divergent self-concepts is a potent initiator of erections in men with psychogenic erectile dysfuncion, has no known side effects and ist very cost efficient.
